TAPE LIFT
The TAPE LIFT is an easy to use test for determining types of visible mold present on a surface. Tape Lift samples can be taken on walls, furniture, or any other surface where mold is present.
After taking the samples, send them to QuanTEM Laboratories. We will analyze them and provide you with a complete report showing the types and relative abundance of each mold present.
CHAIN OF CUSTODY
To make a Chain of Custody, use a blank piece of paper or download a provided Chain of Custody from this web site.
- Fill in your name, address and phone number
- List the sample(s) by number and include the location(s) from which they were taken. Include the address if it differs from your mailing address.
- Be sure to number the samples and list the samples by number on your Chain of Custody so they don't get mixed up.
- Include the name of the person taking the samples.
- Include the date the samples were taken.
NOTE: It is important to include a Chain of Custody with the proper information included. Should you be dealing with your insurance company or a contractor, this information will be required.
MATERIALS NEEDED TO DO A TAPE LIFT:
- Rubber Gloves
- A 2 inch piece of CLEAR tape with one end turned back approximately1/4" (This allows the technician a way to remove the tape.)
- A sandwich bag which can be securely sealed
- Envelope or package to mail sample to QuanTEM Laboratories
- Visible Mold
HOW TO TAKE A TAPE LIFT SAMPLE:
- Place the sticky side of the tape lightly against the mold. Make sure it has made full contact with the mold and run your finger lightly over the tape to insure contact.
- Place the tape inside the baggy so that it adheres to side of the bag. DO NOT stick the tape back to itself. Seal the bag and label it with the sample number. (The numbers can be anything you want to use ~ 1, 2, 3, etc.)
- Complete the Chain of Custody.
- Package up the samples and Chain of Custody.
