BULK SAMPLES

BULK SAMPLES can be taken by removing an actual piece of wall board, or other object on which mold is growing. Physically remove and place the material in a sealable plastic bag. The size of piece required is small and should easily fit into a sandwich bag.

CHAIN OF CUSTODY

To make a Chain of Custody, use a blank piece of paper or download a provided Chain of Custody from this web site.

  • Fill in your name, address and phone number
  • List the sample(s) by number and include the location(s) from which they were taken. Include the address if it differs from your mailing address.
  • Be sure to number the samples and list the samples by number on your Chain of Custody so they don't get mixed up.
  • Include the name of the person taking the samples.
  • Include the date the samples were taken.

NOTE: It is important to include a Chain of Custody with the proper information included. Should you be dealing with your insurance company or a contractor, this information will be required.

MATERIALS NEEDED TO COLLECT A BULK SAMPLE:

  • Rubber Gloves
  • A sandwich bag which can be securely sealed
  • Envelope or package to mail sample to QuanTEM Laboratories
  • Moldy Material
  • Completed Chain of Custody form

HOW TO TAKE A BULK SAMPLE:

  • Using rubber gloves, aseptically remove a sample of moldy material and place it in a sealable, plastic sandwich bag.
  • If multiple samples are being submitted, repeat the previous step using a clean pair of gloves for the collection of each different sample.
  • Seal the sandwich bag(s) and label to match the information on the Chain of Custody.
  • Complete the Chain of Custody. Because bulk materials may be analyzed in one of two ways, you must indicate your choice on the Chain of Custody. Either the sample can be screened to determine the total types of mold present, or it can be cultured to determine the viable types of mold present.

Screening results for total types of mold will be available in 3 -5 days of receipt of the samples at the laboratory. Because mold growth takes time, cultured results will be available in 10 - 14 days of receipt of the samples at the laboratory. The price for each BULK SAMPLE screened is $25.00 and the price for each BULK SAMPLE cultured is $35.00.
